LIABILITY WAIVER & CONSENT TO EMERGENCY MEDICAL TREATMENT
I clearly understand that the C.S. Lewis Foundation (CSLF), in partnership with the C.S. Lewis Foundation UK (CSLF UK), has organized and promoted this event and that they will provide the educational experience as generally described in the informational materials. I further understand that due to the number of speakers, workshop leaders, and other program elements that are based upon the schedules of others, CSLF and CSLF UK shall not be held responsible for program changes, which may be out of their control. Neither shall CSLF or CSLF UK be held responsible for any inadvertent errors, which may have been made in brochures or other promotional information.
I further hereby release CSLF and CSLF UK from any and all claim of liability for injury, illness, accident, and/or loss of property as a result of my participation as a conferee at any C.S. Lewis Conference & Faculty Forum activities. I understand that the values of the C.S. Lewis Foundation regarding the conference are of intellectual hospitality, ‘Mere Christianity,’ humility, and collegiality. Overall, we have organized the conference to reflect the Biblical values of the “fruits of the Spirit” as found in Galatians 5:22-23. Behavior towards other attendees that goes against these values, or any behavior that can be considered aggressive or violent (see list below), will be subject to actions such as, but not limited to,verbal warnings, written warnings, removal from the conference, or other actions deemed necessary by conference organizers to maintain a safe and respectful environment. The following actions are considered unacceptable and are grounds for immediate action: Actual or threatened violence against any participant, including physical attacks or intimidation; Yelling, hostile outbursts, or verbal abuse directed at another participant; Harassment, bullying, or stalking of any individual during the conference; Interference with the safety or well-being of others; Carrying weapons or items intended to be used as weapons, except where permitted by law and declared to organizers in advance ; Any behavior that disrupts the conference or creates an unsafe environment.
I do hereby acknowledge my understanding that I am not a beneficiary of any medical insurance provided by the CSLF, CSLF UK, or their affiliated organizations and do accept full responsibility for any medical expenses that I may incur during the course of my involvement in the C.S. Lewis Retreat. In the event of a medical emergency, I authorize any treatment prescribed by an accredited hospital and/or physician, which is deemed necessary for my wellbeing.
